Connect the Docs, Multi-Specialty Network, Inc.
Physicians who meet eligibility requirements, and support the mission of Connect the Docs, are encouraged to become member-owners of the Connect the Docs Multi-Specialty Network.
In order to become a shareholder you will need to sign a brief Purchase Agreement, a Stock Restriction Agreement and a Physician Participation Agreement. The cost of joining, i.e. purchasing a share of CTD’s stock, is $500.00. Members will also be required to support the continued mission of the Network through annual membership dues of $500.00. Once you have made your initial dues payment, additional dues will not be required until January 1, 2010.
My staff is available anytime to meet with you personally to review the steps necessary to become a member.
