Information and Reporting

At Care Coordinator we are able to provide data in many different forms. We are able to customize our reports to fit any of our customer’s needs. A few examples are shown below depicting orders by organizations referred to, orders by procedure, and orders by procedure by diagnosis. We are able to provide data in many other ways and can even show patient flow through the medical system.

Reports Daily/Weekly/Monthly

Care Coordinator can also provide our customers with reports on a daily, weekly, or monthly basis. Our tracking system allows for our customers to put a dollar value to their increased order volume as a result of using Care Coordinator. We do not direct referrals, but through our efficiency model we are able to help increase the number of orders for our customers, while continuing to provide the highest patient satisfaction

Demographic Report

Through working with our medical facility partners, Care Coordinators has developed standardized demographic reports, which are sent with every referral. These reports contain verified patient data and allow for streamlined registration and scheduling at all medical facilities.

To achieve the same level of efficiency at physician offices Care Coordinator provides customizable referral forms allowing physicians to quickly and clearly enter a referral request and send it to Care Coordinators. Our referral forms are paper and electronic providing physicians the choice to use what works best for them.
