Personal Care Coordinators
Your personal Care Coordinator streamlines individual care and improves your satisfaction by eliminating unnecessary delays you may have previously experienced while caring for your healthcare needs.
The goals of your Care Coordinator are:
- To make your testing process easier by having one person to contact with questions or concerns.
- To improve your experience by eliminating unnecessary delays.
- To ensure you are fully prepared for all requirements and processes associated with outside testing.
During your initial contact with the Care Coordinator Program, you will be provided a Care Coordinator name and a direct phone number. If at anytime you have questions or concerns, contact your Care Coordinator directly and they will be happy to assist you. You should not have to call your physician’s office to inquire about your testing – we are always here to assist you.
Navigating the Healthcare System
We are here to assure the best possible experience, and the best possible outcome. Nothing will stand in the way of providing for your every concern. When you are at your physician’s office and it is determined that you need a particular test or lab performed, the Care Coordinator Program will be notified before you leave the office. Our Care Coordinator will immediately begin the process of taking care of your physician’s request.
Within 24 hours of your visit to your physician, we will contact you regarding the tests your physician has ordered for you. We will provide you with an appointment at the facility specified by your physician. In addition, all pertinent instructions regarding your referral and facility will be thoroughly explained.
Your Care Coordinator will tend to all insurance authorizations and benefits on your behalf, as well as clarify benefits you may have questions about. Directly following your outpatient testing, your Care Coordinator will make certain the results are sent to your physician.

Our team will work closely with insurance companies to ensure your best possible coverage and understanding of benefits. We will follow up with you to confirm your outpatient testing was completed, or in the case of an appointment that was missed, we will assist you with rescheduling your appointment.
